Clinic Policy
Ortho Plus is committed to the success of your medical treatment and care. The following is a statement of our Clinic Policy. This policy applies to all services provided by Ortho Plus, at all of our locations.
Before Your Visit
It is our policy to verify & update patient demographic information each time you visit. As such, you may be asked to update any new information during the check-in process. If you have had any recent demographic changes such as address, phone number, insurance provider, etc. We ask that you provide that information at check-in.
For patients seeking care under MVA billing, the accident claim number is required at check-in.
In the event of any changes to your case, such as legal representation alterations and case settlements, we urge you to notify our corporate office immediately for appropriate updates to your account. If your case is settled, please provide a letter confirming the closure so that we can proceed with any personal health insurance filings.
Insurance policies are a contract between you and your insurance company. We will verify your benefits prior to your first visit. Understanding your benefits associated with your particular plan is vital to knowing your out-of-pocket expense. All co-pays, deductibles, and co-insurances will be collected at check-in. We will also collect previous outstanding balances at this time. As a service to you, we will file your insurance claim. Be aware that there is a time limit on how long we have to file the insurance claims. If we miss the deadline because you did not provide us with the correct information, you will be responsible for payment in full. Once the claim is processed by your insurance, we will bill (or refund if necessary) any remaining balance.
It is your responsibility to make sure that we have your most current insurance information. If you change or add an insurance policy, you must make our staff aware and present a new insurance card at check-in. Any balances not paid by your insurance carrier are your responsibility and payment is due upon receipt of your billing statement.
If your insurance company requires a referral form or authorization, it is your responsibility to obtain this from your primary care provider or insurance company prior to your appointment. We will be happy to assist you with this process. If a referral is needed and not provided, you will be responsible for payment in full. Ortho Plus participates with many, but not all, insurance plans. It is your responsibility to contact your insurance company to verify that we participate with your plan.
Ortho Plus offers competitive cash pricing for our patients with no Medical Insurance Benefits. Ortho Plus requires payment prior to being seen in the practice. If additional services are provided, the remaining balance will be due at the time of service.
Appointment Reminders & Arrival
Ortho Plus utilizes an automated patient reminder system to alert you of your upcoming appointments. Alerts can be in the form of text messages, phone calls, or emails. These reminders are a courtesy and should not be relied upon as your only method of appointment notification.
It is important to arrive on time for your appointments. Appointment times are reserved specifically for each of our patients. Late arrival impacts your own appointment and the appointments of others. While we do our very best to accommodate all of our patients, we do not guarantee the ability to see you if you are more than 15 minutes late. If you are running late, please call our office at 1 (844) 672-2329 well ahead of your appointment time so appropriate accommodations can be made.
Missing an appointment not only affects your health but also creates unused slots for our providers that could benefit other patients. To avoid any additional fees, we kindly request that you notify Ortho Plus at least 24 hours in advance if you are unable to make your appointment.
A lapse of thirty days without a provider visit constitutes a gap in care which may lead to delays and funding issues. If it has been over thirty days since your last visit, it is crucial to communicate this with your attorney and provider.
At the Time of Service
We accept most insurance plans & payment by cash, check, Visa, MasterCard, Discover, and American Express. We do not accept Care Credit. A $40.00 fee will be charged for all checks returned by our bank for non-sufficient funds. Ortho Plus may require a patient to have a credit card on file to pay for any outstanding services.
Ortho Plus has a contractual obligation (with your insurance company) to collect your deductible, co-pay, and co-insurance. These payments are the patient’s responsibility and are due at check-in. Ortho Plus does not bill deductibles, co-pays, or co-insurance. We cannot waive co-pays, deductibles, or coinsurance, for non-covered services defined as patient responsibility under the terms of our contract with health plans.
You are responsible for all lost or misplaced personal valuable items and are therefore encouraged to leave items at home. Ortho Plus is not responsible for the loss or damage to these items in any way.
There is a $25.00 fee when specialized paperwork is required. This must be paid upfront at the time the paperwork is requested. Please allow our office 7 business days to complete your paperwork.
Billing & Financial Obligations
The Ortho Plus Patient Financial Policy outlines our expectations for payment for the services we provide. We are committed to working with you to ensure that your financial obligations are manageable.
If your insurance company does not pay Ortho Plus within a reasonable period, the unpaid balance will become your responsibility. While Ortho Plus may request your assistance in resolving nonpayment issues with your insurance company, ultimately, it is your responsibility to pay for the services provided to you. We are providing medical services to you, not to your insurance company.
You are responsible for all portions of the bill that are denied or not covered by your insurance plan. Account balances are due within 30 days of receiving your billing statement, whether by paper or online. Payment options include:
Balances must be paid prior to your next appointment unless a payment plan has been established. In cases where a payment plan is implemented, a credit card on file will be required.
Accounts with unpaid balances over 60 days may be referred to a collection agency if no payment arrangements have been made. Additionally, if a payment plan has been established and the agreed-upon payments are not made, the account may also be sent to collections. Accounts with unpaid balances over 120 days, where no payment plan exists, will automatically be referred to collections.
Patients with overdue accounts may not receive further services at Ortho Plus unless special arrangements are made. If you are experiencing financial difficulties, please call or contact us as soon as possible to discuss payment options. We prioritize helping our patients create payment plans that work for their financial situation.

Our team is available Monday through Friday, 8 am–5 pm, to assist you. We are happy to discuss payment plans tailored to your financial needs and to help you meet your financial obligations.

Prescription Management
If you perceive that you will run short of medication, please contact our office. The medical provider will be able to review the pharmacy request during weekdays; Monday – Thursday from 8:00 a.m. to 4:00 p.m. so that your medication can be refilled in a timely manner and the appropriate documentation of the use of that medication can be made for our records.
Please evaluate your need for narcotic medication refills on Thursday, as we do not refill narcotic medications on Fridays (unless you have a Friday appointment) or over the weekend.
