Clinic Policy
We are committed to the success of your medical treatment and care. The following is a statement of our Clinic Policy. The policy applies to all services provided by Ortho Plus, at all of our locations.
It is our policy to verify & update patient demographic information each time you visit. As such, you may be asked to update any new information during the check-in process process. If you have had any recent demographic changes such as address, phone number, insurance provider, etc. We ask that you provide that information at check-in.
Our clinic utilizes an automated patient reminder system to alert you of your upcoming appointments. Alerts can be in the form of text message, phone call, or email. These reminders are a courtesy and should not be relied upon as your only method of appointment notification.
It is important to arrive on time for your appointments. Appointment times are reserved specifically for each of our patients. Late arrival impacts your own appointment and the appointments of others. While we do our very best to accommodate all of our patients, we do not guarantee the ability to see you if you are more than 15 minutes late. If you are running late, please call our office well ahead of your appointment time so appropriate accommodations can be made.
We accept most insurance plans & payment by cash, check, Visa, MasterCard, Discover, and American Express. We do not accept Care Credit.
A $25.00 fee will be charged for all checks returned by our bank for non-sufficient funds.
In the event a bill goes unpaid and no prior arrangements have been made with our clinic, your account will be turned over to a collection agency after 120 days. Patients may not be seen for any additional conditions if they have an outstanding account unless special arrangements have been made. If you have difficulty paying your bill, please contact your location. We will make it a priority to help you with a payment paln that works for you.
We will verify your benefits prior to your first visit. Understanding your benefits associated with your particular plan is vital to knowing your out-of-pocket expense. All co-pays, deductibles, and co-insurances, will be collected at the time of service. Once the claim is processed by your insurance, we will bill (or refund if necessary) any remaining balance. It is your responsibility to make sure that we have your most current insurance information. If you change or add an insurance policy, you must make our staff aware and present a new insurance card prior to your appointment. Any balances not paid by your insurance carrier are your responsibility and payment is due upon receipt of your billing statement.
We have a contractual obligation (with your insurance company) to collect your deductible, co-pay, and co-insurance. We will collect these at the time of service. Our office does not bill deductibles, co-pays, or co-insurance. These payments are the patient’s responsibility and are due at the time of service. We cannot waive co-pays, deductibles, or coinsurance, for non-covered services defined as patient responsibility under the terms of our contract with various health plans.
For our patients with no Medical Insurance Benefits, Ortho Plus requires a minimum payment for all self-pay patients prior to being seen in the practice. This is only a minimum payment and the patient will be required to pay the remaining balance in a timely fashion. Payment arrangements can be made through the billing department if the remaining balance cannot be paid in full within 30 days of receiving the patient statement.
